Clear cell renal cell carcinoma

Diagnosis almost certain

Presentation

Let flank pain with palpable mass.

Patient Data

Age: 45 years
Gender: Female

Large well-encapsulated left renal mass of low signal on T1, heterogeneous high signal on T2 with a central area of necrosis and hemorrhage. The postcontrast sequences show a strong enhancement of the peripheral solid component in the arterial phase with washout in the portal phase and prominent surrounding vascular collaterals. There is an extension to the renal sinus with a mass effect on the renal cavities with no extrarenal extension. No abdominal lymphadenopathy. The left renal vein and IVC are patent.

Small intrahepatic arteriovenous shunt (segment 7) with no metastatic lesion seen.

Small splenic cyst.

The right kidney, adrenal glands, and pancreas are normal.
