Case contributed by A.Prof Frank Gaillard

Fall onto head from 3 meters.

Patient Data

Age: 50 years
Gender: Male

CT brain

CT of the brain demonstrates very extensive haemorrhagic contusions particularly over the convexity and inferior surface of the left frontal lobe. This is associated with thin acute subdural and subarachnoid haemorrhage. Significant positive mass effect is already present with midline shift and subfalcine herniation.

An undisplaced comminuted fracture of the occipital bone is noted extending to the foramen magnum, the right hypoglossal canal and to the left mastoid process.

CT brain (next day)

CT obtained the next day demonstrates maturation of cerebral contusions and increased mass effect.

Case Discussion

Typical appearances of extensive contre-coup cerebral contusions.
