Dilated loops of small bowel with two proximal and one distal transition points seen in the right iliac fossa. Reduced enhancement of some loops is concerning for ischemia. Free fluid present in the subdiaphragmatic space, paracolic gutters and pelvis, and engorgement of the adjacent mesentery.
Urgent surgical review advised.
Case Discussion
The patient proceeded to laparotomy, which confirmed closed loop obstruction and small bowel ischemia secondary to an omental band adhesion. Seventy centimeters of non-viable small bowel was resected.
