Presentation

The recurrent headache developed into a thunderclap Headache on the last day.

Patient Data

Age: 35 years
Gender: Male

There is a rim-enhanced, well-defined, rounded iso with central hyperdensity focus mass seen in the roof of the third ventricle obstructing both foramen of Monro and causing dilatation of both lateral ventricles.

Case Discussion

The above imaging findings are consistent with a colloid cyst of the 3rd ventricle causing obstructive hydrocephalus. Colloid cysts are usually nonenhanced; however, they occasionally show subtle rim enhancement as seen in our case. It can be an incidental finding or presenting with a headache as a result of obstruction at the foramen of Monro as seen in our case.
