Presentation

Acute onset severe abdominal pain. Palpable mass in epigastrium.

Telescoping of distal ascending colon and hepatic flexure into proximal transverse colon with a poorly enhancing large mass appearing as a lead point and telescoped inside the distal bowel.

Case Discussion

Intussusception in adults commonly occurs following a lead point. In large bowel, the most common lead point is a neoplasm, commonly colorectal carcinoma. The patient tends to present with severe acute colicky abdominal pain as the peristalsis attempts to relieve the telescoping. Reduction, followed by elimination of lead point is necessary to prevent recurrence.
