Combined femoroacetabular impingement and labral tear

Presentation

Left hip joint pain, decreased mobility. Suspected labral tear - referred for MRI arthrography before arthroscopy.

Patient Data

Age: 50 years
Gender: Male

MR Arthrography

MR arthrography shows an osseous bulge at the anterior aspect of the left femoral head-neck junction, with an 8-mm herniation pit - cam-type femoroacetabular impingement (FAI).
Left acetabular overcoverage of the femoral head - pincer-type FAI.
Full thickness tear at base of anterosuperior labrum, extending posteriorly disconnected but without significant displacement. The tear also involves the cartilage at the transition zone between the anterior labrum and the acetabulum, with a defect that traverses almost the entire cartilage thickness, measuring 4 mm on the coronal plane and 5 mm on the sagittal plane. The posterior labrum is normal.
Subcortical lesion at the posteroinferior aspect of the femoral head measuring 13 mm, displaying intermediate signal on the PD sequence - possibly an enchondroma.
The capsule is preserved.
The femoral ligaments are normal.
