Presentation
Fall on outstretched hand with severe pain and edema around the wrist
Patient Data

Grossly comminuted intra-articular fracture of the distal radial extending through the radial styloid process. Gross dispersion of bony chips at the volar and dorsal aspects of the distal radius.
Carpal rows alignment and joint spaces are preserved.
Soft tissue hematoma surrounding the distal radial fracture. A dorsal slap is noted.
Case Discussion
Comminuted fracture of the distal radius is not uncommon and usually, the mechanism is falling on an outstretched hand.
A close match is a Die-punch fracture especially when comminuted, but it well shows an obvious focal depression of the radial lunate fossa.
