Known cystic fibrosis with bilateral lung transplants. Now diabetic and chronic renal failure with abnormal liver function.

Complete fatty replacement of the pancreas (arrows), i.e. no residual pancreatic parenchyma with marked fatty infiltration of the liver (another common manifestation of CF) and hepatic contour changes indicative of cirrhosis and portal hypertension (also seen in CF).
Case Discussion
There are a number of causes of complete pancreatic lipomatosis:
- cystic fibrosis (most common cause in childhood)
- metabolic/endocrine: hyperlipidemia, diabetes mellitus
- storage disease: hemochromatosis
- drugs: steroids
- infectious: viral infection, sepsis
