Presentation

Twisting injury followed by anterior ankle pain

Patient Data

Age: 20 years
Gender: Female

Complete tear of anterior talofibular ligament is seen with fluid in anterolateral gutter.

Low-grade sprain of posterior talofibular and calcaneofibular ligaments is seen with maintained continuity.

Anterior tibiofibular, posterior tibiofibular and interosseous ligaments appear intact.

Low-grade sprain of deep fibers of deltoid ligament is seen with subtle marrow edema in medial aspects of adjacent talus.

Mild edema is seen in sinus tarsi with possible low to intermediate grade sprain of cervical and interosseous ligaments.

Case Discussion

The images depict complete tear of anterior talofibular ligament which is the most common ligament injury of ankle. It is commonly associated with tears of calcaneofibular ligament; which in this case is intact and very well seen.
