Presentation
Facial trauma and progressive left upper eyelid swelling and left eye proptosis and hypoglobus.
Patient Data
Left orbital roof blow-in fracture extended within orbital upper rim and frontal sinus outer table. There is also a left orbital floor blow-out fracture with an orbital fat hernia within the fracture gap. Upper orbital cavity postseptal emphysema and eyelids emphysema more prominent in upper eyelid are also noted. Impression of displaced fractured bones on the left eye and superior rectus muscle, proptosis, and hypoglobus are seen. Small osteoma coronal width up to 7 x 5 mm in the right frontal sinus outflow tract is noted. Fluid and thick mucosa in paranasal sinuses are seen.
Case Discussion
The case illustrates the rare concomitance of the orbital roof blow-in and orbital floor blow-out fractures complicated by proptosis and hypoglobus, eyelids, and orbital emphysema. One of the major complications of the orbital roof fractures is dural tear and CSF leakage.
