Congenital absence of internal carotid artery

Diagnosis almost certain

Presentation

A patient with diabetes and hypertension is complaining of dizziness, requiring investigation to rule out an acute stroke.

Patient Data

Age: 65 years
Gender: Female

No restricted diffusion to suggest acute infarction.

Brain MRA shows an absent left internal carotid artery.

Mild small vessel disease is seen in T2 images.

A previous study (not shown) showed loss of signal void of the left internal carotid artery, raising the possibility of the left internal carotid artery agenesis rather than chronic occlusion.

Case Discussion

The patient is complaining of dizziness. The MRI study reveals an incidental finding of an absent left internal carotid artery without acute infarction in both this study and previous one.
