Diagnosis certain

Presentation

Pectus excavatum and increased work of breathing.

Patient Data

Age: 11 weeks
Gender: Female
x-ray

There are multiple gas filled bowel loops in the anterior left hemithorax, consistent with a morgagni type of diaphragmatic hernia with resultant mediastinal shift to the right. The partially visualized superior left lung is clear. The right lung is clear.  In addition, there is a pectus excavatum, best appreciated on the lateral view.

Case Discussion

This was a postnatal diagnosis of a congenital diaphragmatic hernia. There was no history of antenatal ultrasound examinations, which would likely have picked the anomaly in utero.
