Age: 1 day
Gender: Male
x-ray

Chest and abdominal radiograph for line placement. 

ET, NG, UVC and UAC. Appropriate positions.

Left diaphragm is absent. Lucencies projected above the expected position of the diaphragm. NG tube displaced to the right. Cardiac silhouette is also displaced to the right.

Appearances are of a congenital diaphragmatic hernia.

1 week after repair

x-ray

After repair there has been expansion fo the left lung which will be hypoplastic. The initial pneumothorax has resolved and pleural fluid has started to collect as expected.

Case Discussion

Laparotomy confirmed a congenital diaphragmatic hernia. After bowel was puled down from the chest, the posterior defect in the diaphragm was closed by primary repair. No mesh was required.
