Homogenous well-defined right lower thoracic paracardiac opacity.
The right kidney is located above the right diaphragm with good vascularization, no hydronephrosis.

Case Discussion
Intrathoracic kidney is a rare congenital anomaly, but it should be included in the differential diagnosis of the well-defined lower thoracic opacity, the diagnosis can be easily confirmed by ultrasound in most cases as we can use the acoustic window of the liver or spleen to demonstrate it above the diaphragm.
