Case contributed by Yusra Sheikh
Diagnosis certain

Presentation

Enlarging bump over the right clavicle.

Patient Data

Age: 8 years
Gender: Male

There is a superiorly angulated abnormality of the mid shaft of the right clavicle, with well sclerosed adjacent margins, most suggestive of congenital pseudoarthrosis of the clavicle.

Six years prior (2-year-old)

x-ray

There are two separate components of the right clavicle with smoothly outlined adjacent surfaces, most suggestive of congenital pseudoarthrosis of the clavicle.

Case Discussion

The second radiograph above was performed when the patient was 2 years old with a lump over the right clavicle, which the patient's parent reported as having been present since birth. Congenital pseudoarthrosis of the clavicle, is most common on the right as our case demonstrates.

A fracture of the clavicle in an infant may be suggestive of non accidental injury (inflicted injury). A careful clinical history and examination in this age group is therefore  important.
