Age: 5 years
Gender: Male
x-ray

Spontaneous pneumomediastinum with typical continuous diaphragm sign is present. 

Spontaneous subcutaneous emphysema also noted. 

Case Discussion

Repeated cough can result in spontaneous pneumomediastinum and subcutaneous emphysema. Macklin effect could represent the pathophysiology behind the case findings.
