Diagnosis certain

Presentation

Antenatally detected interhemispheric cyst under evaluation.

Patient Data

Age: 3 months
Gender: Male

MRI brain of the neonate shows complete absence of corpus callosum with colpocephaly (parallel arrangement of dilated occipital horns of lateral ventricles) and large CSF intensity thin-walled cyst in the interhemispheric region (to the right of falx). Cyst measures 5.4 x 2.9 x 5 cm (AP x TR x CC) in maximum dimensions. No restricted diffusion was seen. No obvious communication of the cyst with ventricles was noted.

These features are consistent with complete corpus callosal agenesis with interhemispheric arachnoid cyst.

Case Discussion

Complete absence of corpus callosum with anterior interhemispheric cyst in rare congenital anomaly. The interhemispheric cyst is usually in paramedian location and may contain loculations.
