Right frontotemporal contusions with mild subfalcine herniation and small associated SDH. There is a tiny amount of blood within the interpeduncular cistern, and also layering along the left brachium pontis.
The left parietal soft tissue hematoma and underlying skull fracture (without EDH) indicate that the traumatic impact was left posterior, and consequently the right frontotemporal hemorrhages represent a contre-coup injury.

This case demonstrates a typical coup-contre coup pattern of injury, with an extracranial/skull injury at the point of direct impact, and a considerably more pronounced area of intracranial contusions at the directly opposite side of the head.
