Presentation
Fall from the second storey.
Patient Data
Right frontotemporal contusions with mild subfalcine herniation and small associated subarachnoid hemorrhage. There is a tiny amount of blood within the interpeduncular cistern, and also layering along the left brachium pontis.
The left parietal subgaleal hematoma and underlying skull fracture (without EDH) indicate that the traumatic impact was left posterior.
Case Discussion
This case demonstrates a typical coup-contrecoup injury pattern, with an extracranial/skull injury at the point of direct impact, and a considerably more pronounced area of intracranial contusions at the opposite side of the head.
