Presentation

High-grade fever and productive cough and chest pain.

There are scattered patchy-nodular ground glass densities in both lungs and the one located in the posterolateral right lower lobe has the features of a faint reversed halo.

Case Discussion

The two main differential diagnosis of reversed halo sign in pulmonary infections is COVID-19 and mucormycosis at the moment. The nasopharynx PCR test was positive for COVID-19 in this non-diabetic patient. The point is that COVID-19 pneumonia can be also complicated by secondary mucormycosis infection, especially in paranasal sinuses which is more commonly seen in diabetic patients. The reversed halo sign is non-specific and can be also seen in other conditions such as active pulmonary tuberculosis and cryptogenic organizing pneumonia.
