Case contributed by Dr Domenico Nicoletti

Presentation

Marked dyspnea with dry cough and high fever for about nine days

Patient Data

Age: 50 years
Gender: Male
X-ray

Chest

Examination within the limits of the norm.

On the same day, the patient performed a chest CT scan.

CT

Chest

There are large bilateral pure ground-glass opacities in the upper and lower lobes prevalent in the dorsal region, with peripheral distribution and slightly dilated vessels in the affected area. Ground-glass opacities with a rounded morphology are also present in the middle lobe and lingula.

On the right, there is a modest amount of pleural effusion in the inter-lobar fissure with thin fibrous bands in the medial segment of the ipsilateral lower lobe. No pathological adenopathies.

Pathology

Nasopharyngeal swab report

Nasopharyngeal swab report revealed positive for Coronavirus SARS-CoV-2.

Case Discussion

The chest film may be insensitive at the beginning of the disease. The main CT findings in pneumonia COVID-19 are ground-glass and consolidative pulmonary opacities, and these findings are more frequent with longer time from symptom onset.
