Cowper duct syringocele

Presentation

The patient presented with urinary frequency and weak stream.

Patient Data

Age: 65 years
Gender: Male
mri

There is an ovoid cystic structure in the midline posterior aspect of the bulbous urethra, which appears hyperintense on T2 and shows peripheral enhancement, measuring about 17.5 x 6 x 4.5 mm (AP.TRANS.CC), consistent with Cowper duct syringocele​.

Enlarged prostate with few well-defined encapsulated lesions in the transitional zone are noted suggestive of BPH.

Intact prostatic capsule.
Intact neurovascular bundles bilaterally.
No peripheral zone masses.

Case Discussion

There are two causes of bladder outlet obstruction in this case, the prostatic enlargement and the Cowper duct syringocele.
