Presentation

Skull bony exostosis/spur.

Patient Data

Age: 25 years
Gender: Female

Brain

ct

Large left medullary ground glass density with bony expansion seen involving the frontal bone and extending inferiorly involving the roof and part of the medial and lateral walls of the left orbit. Involvement of the ethmoidal bone more on the left and crista galli. The lesion is causing mass effect on the left anterior frontal lobe without significant midline shift.

No hydrocephalus.

Both optic foramina are seen without significant narrowing. No evidence of retrobulbar masses.

Case Discussion

In craniofacial fibrous dysplasia, the affected bones are usually expanded with an intact cortex, and the medulla been replaced classically by a homogeneous ground glass appearance, although mixed lucencies and sclerosis are also common. The anterior craniofacial bones are more frequently involved than more lateral or posterior portions. Commonly involving the paranasal sinuses, nasal cavity, orbits, sphenoid, frontal, maxillary, and ethmoidal bones.
