There is a voluminous, polylobulated, surpra-sellar mass hyperintense on T1 and T2 (with intra-sellar extension, since it is big!), without any solid component. Post-gadolinium T1 show no enhancement since it is a cyst with high protein content. One part of the lesion is actually purely cystic, located at the upper third on the right. Compression of the optic chiasm is noted.
This child actually presented with visual symptoms.
Differential diagnoses include adamantinomatous craniopharyngioma vs. Rathke's cleft cyst.
