Crohn disease presenting as small bowel obstruction

Case contributed by Dr Chris O'Donnell

Presentation

Past history of Crohns disease. Now abdominal pain and vomiting. Bowel obstruction or perforation?

Patient Data

Age: 25
Gender: Male

Marked fluid-filled dilatation of small bowel loops with abrupt transition to uniformly thick-walled terminal ileum showing "fat halo" and "comb" signs indicative of inflammatory bowel disease.

Case Discussion

It is unusual for an acute flare-up of Crohns disease to cause mechanical small bowel obstruction rather pain and diarrhoea.  More chronic Crohns disease is associated with bowel obstruction due to the development of fibrous strictures.
