Crohn disease presenting as small bowel obstruction

Presentation

Past history of Crohns disease. Now abdominal pain and vomiting. Bowel obstruction or perforation?

Patient Data

Age: 25
Gender: Male

Marked fluid-filled dilatation of small bowel loops with abrupt transition to uniformly thick-walled terminal ileum showing "fat halo" and "comb" signs indicative of inflammatory bowel disease.

Case Discussion

It is unusual for an acute flare-up of Crohns disease to cause mechanical small bowel obstruction rather pain and diarrhea.  More chronic Crohns disease is associated with bowel obstruction due to the development of fibrous strictures.
