Presentation

Chronic abdominal pain and loss motion.

Patient Data

Age: 40 years
Gender: Male
ct

There is marked wall-irregularity, mild mural thickening of the cecum and ascending colon with surrounding significant fibrofatty proliferation.
Small collection with internal air bubbles are seen in the right paracolic gutter 7 x 5.5 x 3.5 cm communicating with the ascending colon.
Rest of the bowel loops show normal CT features. 
Pelvic ascites is also noted.

Case Discussion

The features are suggestive of inflammatory bowel disease (likely Crohn's disease) with perforation of ascending colon and local abscess formation as well as pelvic ascites.
