Crutchfield carotid artery clamp

Case contributed by Dr Jayanth Keshavamurthy

Cough. A chest radiograph was performed for evaluation of pneumonia.

Patient Data

Age: 90
Gender: Female

Can you identify all the surgical changes and foreign bodies?

Modality: X-ray

Patient is status post left mastectomy. Multiple surgical clips overlying the left axilla. There is a moderate right pleural effusion and biapical pleuroparenchymal scarring.

Embolization coils are seen overlying left upper abdominal quadrant. A Crutchfield type carotid
artery clamp is identified within the left inferior cervical soft tissue, overlying the left common carotid artery.

Modality: CT

Carotid artery clamp in left carotid artery.

Case Discussion

Crutchfield clamps were once widely used for proximal carotid artery ligation in the treatment of intracranial aneurysms and cavernous sinus fistulae. A radiologist needs to be aware of this entity as there may be early or delayed complications of long standing iatrogenic foreign bodies.
