Presentation
Cough, evaluation of pneumonia
Patient Data
Can you identify all the surgical changes & foreign bodies?
Three chest radiographs are provided, in order: 120 KV, dual energy subtracted and lateral.
Status post left mastectomy. Multiple surgical clips overlying the left axilla. There is a moderate right pleural effusion and biapical pleuroparenchymal scarring.
Embolization coils are seen overlying left upper abdominal quadrant. A Crutchfield type carotid artery clamp is identified within the left inferior cervical soft tissue, overlying the left common carotid artery.
Carotid artery clamp in left carotid artery.
Case Discussion
Crutchfield clamps were once widely used for proximal carotid artery ligation in the treatment of intracranial aneurysms and cavernous sinus fistulae. A radiologist needs to be aware of this entity as there may be early or delayed complications of long standing iatrogenic foreign bodies.
