Case contributed by Dr Mohamed Hossam el Deen

Parent complaining they do not feel their baby's testicles since birth.

Patient Data

Age: 2 months
Gender: Male

MRI sequecences show bilateral two ovoid shaped masses along the path of testicular descent parallel to the course of gonadal vessels at the inguinal canal level.

They are of intermediate signal in T1 and homogenous high signal in T2 and T2 fat sat. The left testicle appears more atrophied.

Empty scrotum is noted in all MRI sequences.

Case Discussion

MRI is more sensitive than ultrasound in detection of cryptorchidism and its level. Undescended testicle appears typically iso intense in T1 and homogenous hyper intense in T2, T2 fat sat and STIR.

Usually contrast is not needed for detection. Atrophic changes of the testicle ( like decrease in size with decreased T2 signal intensity ) and any associated penile deformities should be excluded.

Testicles also exhibit strong diffuse restriction in MRI if these sequences feature in local protocol.
