Case contributed by Bálint Botz
Diagnosis certain

Presentation

Sudden onset dysarthria and vertigo. CT with multiphase CTA and CTP negative.

Patient Data

Age: 50 years
Gender: Female

5 minute stroke protocol ultrafast MRI shows a small but unequivocal focus of restricted diffusion, bordering the left cerebellar vermis. It is not yet matched on FLAIR.

Case Discussion

Rapid stroke protocol MRI excels at the early depiction of small ischemic foci. The absent FLAIR signal abnormality indicates that this is indeed an acute ischemic lesion.
