There is signal alteration in the cerebellum, occipital lobes, putamina, caudate nuclei (head), centra semiovalia, corona radiata bilaterally, and the splenium of corpus callosum, with hyperintensity on FLAIR and T2W, diffusion restriction, and hypointensity on T1W. No haemorrhage was detected.

Case Discussion

Cyanide was detected in the stomach content of this patient.

Cyanide poisoning characteristically tends to affect the structures that have the highest oxygen requirement, and are therefore the most sensitive to oxygen deprivation: the basal ganglia and the cerebral cortex. Acute cyanide poisoning can cause rapid deterioration and death. Luckily, this patient healed after few weeks.
