Presentation

Right knee chronic pain after ACL reconstruction aggravated by recently direct knee trauma.

Patient Data

Age: 25 years
Gender: Male
mri

Joint effusion.

Abnormal intrasubstance increased fluid signal and fiber discontinuity along the course of the ACL graft intra-articular part related to the ACL graft partial tear.  PCL buckling.

Prominent scarring anterior to the ACL graft projecting into the inferior aspect of Hoffa's fat pad measuring about 13*22*25 mm.

Bone marrow edema/bruise is present at the posterolateral tibial plateau and patella medial pole.    

Case Discussion

Postoperative scarring related to ACL repair which is compatible with cyclops lesion (also known as localized anterior arthrofibrosis).
