Presentation

Progressive dyspnea and recurrent infections since suffering pneumonia 13 years ago. On home oxygen.

Patient Data

Age: 55 years
Gender: Male
x-ray

Innumerable cystic structures sparing the periphery.
Numerous ring and tram track shadows.
Large volume lungs with flattened diaphragm bilaterally.

Widespread bronchiectasis sparing the major airways and periphery.
Mosaic attenuation.
Areas of atelectasis with crowded bronchi.

Pathology summary:

1. Right explanted native lung: Features of marked severe chronic bronchiectasis with adjacent changes within the alveolar parenchyma and more peripheral/subpleural mild emphysematous and fibrotic disease.
2. Left explanted native lung: Features of marked severe chronic bronchiectasis with appropriate changes within the alveolar parenchyma and more peripheral/subpleural mild emphysematous and fibrotic disease.

Case Discussion

No evidence of immune deficiency or cystic fibrosis. Cystic bronchiectasis in this case is assumed to be a consequence of a severe bout of pneumonia.

He remains well several years following bilateral lung transplant.
