Diagnosis probable

Presentation

Left cervical painless swelling, progressively increasing in size over the last 5 years.

Patient Data

Age: 25 years
Gender: Female

Large well-defined oblong multiseptated cystic lesion of the left cervical region displacing the sternocleidomastoid (SCM) laterally and common carotid/internal jugular vein medially.

Well-defined multiseptated cystic lesion of the left cervical region, measuring 95 x 40 x 30 mm, extending from the submandibular region up to the medial supraclavicular region between the sternocleidomastoid (SCM) muscle and left thyroid lobe. It elicits a low signal on T1, high signal on T2 with a low signal of the internal septae. Moderate peripheral enhancement is noted mainly of its upper part with no enhancement of the internal septae. The SCM muscle is displaced laterally and carotid vessels and internal jugular vein medially.

Case Discussion

Ultrasound and MRI features of a multiseptated left cervical cystic mass suggestive of a cystic lymphangioma.

The main differential diagnosis is branchial cleft cyst: The ultrasound features and location are atypical for the 3rd or 4th branchial cleft cyst.
