Cystic lymphangioma with intracystic hemorrhage

Presentation

Progressive right cervical swelling.

Patient Data

Age: 4 years
Gender: Female

The MRI sequences demonstrate a multiloculated, multiseptated cystic mass measuring 47 x 45 x 17 mm, centered on the right posterior triangle and extending to the carotid space with lateral displacement of the SCM and trapezius muscles. It displays a high signal on both T1 and T2 with a sediment of a low signal indicating an intracystic hemorrhage. The postcontrast sequences show a heterogeneous enhancement mainly of the septa.

Case Discussion

The MRI features are most consistent with a cystic lymphangioma, also known as cystic hygroma complicated by intracystic hemorrhage.

 Internal bleeding and infection are the main complications of cystic hygroma/lymphangioma.
