Case contributed by Claire Cuscaden
Diagnosis certain

Presentation

Chronic left sided epiphora.

Patient Data

Gender: Male

Dacroscintigraphy performed following topical eye drops containing Tc99mO4- (Pertechnetate).

Dynamic planar imaging performed to 60 minutes with SPECT/CT performed of the facial bones.
The dynamic imaging demonstrates free drainage of radiotracer from the right orbit into the right nasolacrimal duct. There is no evidence of transit of radiotracer from the left orbit.

SPECT/CT confirms a 10 x 14mm left sided dacryocystocele and no radiotracer identifiable within the nasolacrimal duct or nasal cavity. Radiotracer is confirmed within the right nasolacrimal duct, inferior nasal meatus and pharynx.
