De Quervain's tenosynovitis

Presentation

Pain over the radial styloid process. Mild ROM restriction in the thumb.

Patient Data

Age: 25 years
Gender: Male

1st wrist extensor compartment

ultrasound

Longitudinal and transverse projection showing circumferential thickening of the tendon sheath combined with a thickened pulley in the first extensor compartment of the wrist. Abductor pollicis longus and extensor pollicis brevis tendons are mildly abnormal with local minimal loss of normal fibrillary pattern and trace thickening.

Case Discussion

The findings described above are clearly pointing to the diagnosis of stenosing tenosynovitis of the 1st wrist extensor compartment with some changes indicating minimal tendinosis of the abductor pollicis longus and extensor pollicis brevis tendons.
