Diffuse hyperdensity of internal cerebral veins bilaterally along with the vein of galen and the straight sinus. The luminal thrombus is seen on post contrast study (deep cerebral vein thrombosis).
Hypodensity is noted along the left thalamus as well as both caudate nuclei, more on the left side… In keeping with venous infarcts.
Hypercoagulable state is a recognized predisposing factor for venous thrombosis, so postpartum period is likely to be a predisposing factor in this case. Deep cerebral vein thrombosis is usually associated with changes of basal ganglia such as edema, venous infarction and hemorrhage. CT venogram is most sensitive and the thrombus appears as filling defects in the veins involved.
