Deep forearm lipoma

Diagnosis probable

Presentation

Partial right arm radioulnar palsy. Extensor carpi radialis longus and brevis functional.

Patient Data

Age: 45 years old
Gender: Male
mri

Large lobulated lipomatous lesion adjacent to the radius and in the proximal forearm causing displacement of the radial nerve. There appears to be a subtle area of T2 weighted hyperintensity in the superior aspect of the lesion, however no abnormal T2 weighted hyperintensity or enhancement along the radial nerve.

Minor fatty atrophy and edematous changes in the supinator, extensor carpi radialis longus, brevis, extensor digitorum and extensor carpi ulnaris.

Case Discussion

Although likely to be a lipoma, consideration of the large size and deep location of the lesion raises the possibility of a liposarcoma.
