Degenerative TFCC - Palmer 2c

Presentation

Ulnar sided pain in the left wrist after injury 11 months ago.

Patient Data

Age: 50
Gender: Female

The radial aspect of the TFC proper is mildly thinned with a small central perforation and contrast in the extravasation into the distal radioulnar joint. The adjacent articular cartilage is intact. 

There is gadolinium signal along the extensor carpi radialis tendons due to inadvertent injection during the procedure.  The tendons of the extensor compartments are otherwise intact.

Case Discussion

Degenerative thinning and perforation of the TFC is typical of a Palmer 2C grade injury. If the central avascular portion of the TFC was not thinned, this would be a Palmer 1A injury.
