Presentation

Crush injury.

Patient Data

Gender: Female
x-ray

Extensive traumatic injury of the foot with separation of the skin/soft tissues of the toes from the bones with some associated avulsion injuries of the distal phalanges of toes 1, 2, and 5. Notice how the tip of the great toe (1st distal phalanx) is substantially separated from its expected location near the tip of the great toenail, where there are only a few fine avulsion fragments. Smaller avulsion fragment at the lateral base of the first distal phalanx. Displaced fractures of the 1st (comminuted, overriding), 2nd, and 3rd metacarpals.

This extensive degloving injury of the foot with separation of the skin and soft tissues of the toes from the bones occurred as a result of a 3000lb industrial object crushing a foot while wearing steel-toed footwear.
