Dense hilum sign - lung cancer

Presentation

Shortness of breath.

Patient Data

Age: 80 years
Gender: Female
x-ray

Dense hilum sign seen on the frontal projection with a right middle lobe mass evident on the lateral. 

Two years prior

x-ray

Lungs are clear. Normal cardiac silhouette. 

Large mass at the anterior aspect of the right hilum extending into the right middle and upper lobes. Distal atelectasis in the right middle lobe. Mediastinal lymphadenopathy. 

Case Discussion

This patient had a subsequent diagnosis of lung cancer (histopathology unknown) and underwent radiation treatment. The dense hilum sign is one to be aware of on frontal radiographs, as a mass can easily be overlooked. In this case, however, it is readily localized on the lateral and when compared to the prior the difference in density of the right hilum is apparent.
