Dense left middle cerebral artery (MCA) extending from the carotid terminus likely represent thrombus. Large left middle cerebral territory infarction without hemorrhagic conversion. Minimal midline shift.

Typical appearance of a hyperdense MCA sign with a left MCA infarction. The most likely source for the thrombus is the heart (left atrial appendage or left ventricle) or carotid artery with embolization into the MCA.
