Case contributed by Ali Abdullah Obaid
Diagnosis almost certain

Presentation

Left-sided lower jaw pain, swelling, and tenderness with a history of dental extraction.

Patient Data

Age: 20 years
Gender: Female
ultrasound

There is a left-sided buccal and fascial space abscess, showing a loculated, turbid collection measuring 1.7 × 0.7 cm, with a track extending deeply to the overlying mandibular bone, where a cortical bone defect communicates with the abscess as a sinus tract formation.

Case Discussion

With a history of dental caries and extraction, ultrasound images revealed a left buccal and fascial space abscess, likely due to an abscess at the root of the tooth (dental abscess).

DDx: odontogenic osteomyelitis with overlying soft tissue abscess formation.

Recommend a CT of the head for further evaluation of the mandibular bone.
