Dental fracture and retained root

Presentation

Alleged assault. Left temporal and orbital tenderness and avulsed tooth.

Patient Data

Age: 20 years
Gender: Male

Fracture of the 21 tooth, with residual root fragment remaining in the socket. The fractured 21 tooth is located at the buccal aspect of 47 and 48. The anterior and posterior alveolar margins of 21 appear intact. Overlying soft tissue swelling and soft tissue gas, consistent with history of trauma. No further facial bone fractures are identified. No skull fracture identified.

Air fluid level within the right maxillary antrum, and partial opacification of the remainder of the paranasal sinuses. The orbital contents appear within normal limits. The mastoid air cells appear well aerated.

Case Discussion

Isolated dental fractures are often overlooked, but with this history the diagnosis was easy. It is important to identify any corresponding dentoalveolar fracture which may change the treatment.
