Dermatomyositis

Presentation

Patient will need assistance on and off table. Dermatomyositis - recently commenced on Prednisolone. Moderately severe proximal limb girdle weakness and moderate dysphagia. Extent of inflammatory muscle disease?

Patient Data

Age: 30 years
Gender: Female
MRI

Extensive symmetrical high T2 signal on FLAIR throughout all the muscles groups of the pelvis and thighs.  Slight sparing of the superomedial aspects of the gluteus maximus muscles.

Minor subcutaenous edema laterally at the level of the hips.

Ostetitis condensans ilia.

Muscle enzymes

Creatine Kinase *810U/L (normal<165)

 AST *73U/L   (normal <40)

Case Discussion

This case shows a moderate-severe dermatomyositis of non malignant etiology.

The laboratory findings and clinical presentation correlate with the MR findings.
