Derotational osteotomies with blade plates

Case contributed by Dr Matt A. Morgan

Presentation

History of spina bifida and intoeing gait.

Patient Data

Age: Child <10Y
Gender: Female

Bilateral blade plates through the proximal femurs and linear subtrochanteric sclerosis from healed fractures are compatible with old derotational osteotomies.

The 10 mm blade plates were bent to 110 degrees.

Case Discussion

Derotational osteotomy is a strategy for treating intoeing in paediatric patients with femoral anteversion. The proximal (subtrochanteric) femurs are surgically fractured and then rotated to a more functional angle. The fractures are then fused with fixation hardware, in this case, angled blade plates.
