Presentation

Mastalgia and bilateral breast pain.

Patient Data

Age: 40 years
Gender: Female

The breasts are dense on mammography.

ultrasound

The ultrasound images are quite startling in real time. Widespread areas of poorly defined hypoechoic mass-like structures with very significant acoustic shadowing is seen in both breasts. The images are almost discrepant compared with the mammogram and are quite alarming on first glance. The acoustic shadowing is very obvious on real time imaging.

Case Discussion

This patient had a long history of type 1 diabetes. The diagnosis of diabetic mastopathy was confirmed on core biopsy of one of the lesions.

This is a rare complication of diabetes and part of the key to diagnosis is a high index of suspicion.
