The MRI sequences demonstrate:
mild scoliosis of right-sided convexity
congenital block vertebra of L4-L5 with butterfly vertebrae
diastematomyelia type I at L4-L5 (hemicords separated by a bony spur)
low-lying spinal cord terminating at L5-S1
hydromyelia at L2-L3 level
spina bifida occulta at several levels
moderate dural ectasia with sacral perineural cyst
Case Discussion
MRI features of a diastematomyelia type I which is the classic form characterized on imaging by:
- duplicated dural sac/cord with midline spur (osseous or fibrous)
- scoliosis or signs of tethered cord syndrome
- hydromyelia common
- vertebral abnormalities
