Presentation

Dysmenorrhea.

Patient Data

Age: 50 years
Gender: Female

The uterus is enlarged with diffuse thickening of the junctional zone ranging from 15 to 30 mm, mainly corporeal anterior. It is of intermediate signal intensity on T1WI, and homogeneous low signal intensity on T2WI with foci of high signal on T1WI within,  indicating menstrual hemorrhage into the ectopic endometrial tissues.

A small unilocular cyst is noted within the right ovary as well as minimal effusion in Douglas pouch.
